Equine Wellness Packages

TEVA’s goal is to take horse care to new levels with a concentrated focus on preventative care for each horse; based on the horse’s needs, and the desires of their owner.  Therefore, we would like you to seriously consider our 4th Annual Wellness Membership.  There are several benefits to enrolling in our Wellness Program, such as:

• Distinct program levels to meet the needs of our varied patients.
• Bundled services at a reasonable discount.
• Includes Coggins, vaccines, exams, blood work analysis, and fecal parasite egg counts.
• Valuable discounts on dentals and sheath cleaning.
• Lyme Disease: enhanced testing and vaccinations.
• "Refer a New Wellness Member Incentive": $25 credit to both referrer and referee.
• 10% discount on all other equine services throughout the year *(e.g., illness and lameness, medications are not included).

              
We are pleased to offer a Performance Package Upgrade this year.  This is an exciting program because it provides you with discounted lameness services aimed to optimize your horse’s performance throughout the pre- and post-show season.  It is an elective upgrade that can be added to your membership.  Package services include:

• Soundness performance examination, includes lunging and flexion testing.
• Candid counseling on performance expectations and the means to achieve your goals.
• Discounted digital radiographs (-15%) for 2012.
• Discounted joint injection maintenance program (-15%).
• Discounted shockwave maintenance program.
• Adequan/Legend/Pentosan/PentAussie/Polyglycan therapy options based on your equestrian goals.
• Nutrition evaluation.
• Fitness and sports rehabilitation plans.  Show season medication considerations.
• Equioxx/Previcox and Tildren therapy options.

These plans are intended to offer services in bundled pricing packages to achieve excellence in your horse's health and performance.  This Membership is intended for clients seeking the most comprehensive, all-inclusive, "Total" care available.  Please call us for more information or see below.

We strive to provide our patients with the best possible care at a reasonable cost.

Gold Membership

The Gold Membership is designed for optimum care.  Equine athletes as well as horses that have an increased risk of disease due to boarding, showing, and traveling are perfect candidates for this Membership.  This membership offers the most comprehensive coverage which includes twice yearly vaccinations as well as blood work and analysis (CBC / Chemistry), Coggins, and twice yearly physical and fecal exams.  This plan includes a dental exam, and additional dentistry^^ as needed at discounted cost if elected by the client.

Silver Membership

The Silver Membership represents the level of care recommended for most other horses.  This membership includes twice yearly exams, vaccinations, a Coggins and a dental exam, with dentistry^^ as needed at additional discounted cost if elected by the client.  This plan is recommended for non-performers or our geriatric/older patients not desiring the Gold Membership.

**Must be scheduled with Visit #1 or #2.

^^ Dental floats do not necessarily occur annually.  Dental floats and sedation will be performed, and charged, as needed, based on patient history and clinical signs.  If dentistry services are needed, the charges will be billed separately at the time of service.  If greater than 24 months since the last dental float, clients may be charged an extensive / initial dental float ($155).  ** Must be scheduled with Visit #1 or #2.
Remember, member horses receive a 10% discount on ALL other TEVA equine services, including emergencies*, throughout the year for all member levels, (NOT including medicines).

Terms and Conditions:  Total Equine Veterinary Associates Annual Wellness Membership

Must enroll (or renew) before May 30th.   We accept cash, checks, all major credit cards, and PayPal for your pre-payment convenience.

TEVA will contact you to schedule Wellness appointments.  This schedule will be based on geographic area if possible to save both time and fuel.  TEVA will monitor and inform you of your horse’s individual progression throughout the Membership, ensuring services are not missed.  There is no carry over from previous years.  TEVA reserves the right to modify or cancel this program at any time. 

Continued enrollment requires annual renewal.  Your selected care level may not be changed during the year of enrollment.  Membership Level may be changed during annual renewal.  If an enrolled horse passes away or is sold while participating in the Wellness Membership, a prorated refund or credit will be given, or you may transfer Membership to the new owner, or you may transfer Membership to another horse.

10% discount offered for all other equine services only applies to the enrolled animal when seen by a TEVA employee.  Individual horses are the intended members, not the owners.  Emergency services provided by outside veterinarians will not be discounted.  We participate in a 24/7 emergency on-call rotation with other local equine veterinarians and cannot offer discounts on their services.

The following services are not pre-paid in the Wellness Program but will be discounted 10%: farm call fee, initial 30 day booster vaccines.  Participants may split farm call fees.   

“Refer-a-New Wellness Member Incentive” applies $25 TEVA account credit to both the referrer and the new client when both enroll in the Membership and both Memberships are paid in full.
Minor deviations from the Membership Plans are permitted.
